         The purpose of this interview is to investigate claims made by ______ in a lawsuit that has filed against ________.

         I am interested only in truthful replies to my questions.

         There will be no reprisals against you as a result of your answers.

         I will be taking notes.  I will show my notes to you at the close of our visit for you to review and make any corrections, additions, or other revisions to make sure that my notes are 100 percent accurate.

         To the extent possible, I will keep my interview notes of you confidential.  However, if you are called to testify in this matter and testify to facts that are inconsistent with what you tell me today, these notes will become relevant and may be used during the litigation or at trial.
